PmWiki already makes sure that every <img> tag ends up with an alt=
attribute for html validation, even if it's just alt="".

This is by far the best and most consistent formulation of 
title and alt text handling that I've come across.  Excellent.

Note that the caption option only exists for images that
start a line; there's not currently a way to get captions for
inline images.  By similar considerations there wouldn't be a 
way to add hover text onto an inline image except by using a
[[...]] link of some sort.  But perhaps that's not too big
a limitation.
