Since I posted the above, I've found a few orphaned skins that weren't listed
in the main skins list. They're listed now. Most of them actually were
activated, but one wasn't. The opposite problem was far more common: There are
a fair number of skins installed on the main PmWiki site (as listed by
skinlist: http://www.pmwiki.org/wiki/Cookbook/SkinList ) which don't have
Cookbook pages. In some cases I'm sure its deliberate. I'm not sure about the
others.

Anyway, add this one to the above list of skins to activate (I've tried it. It
needs updating to support farms, but otherwise works).
