Did you set $UploadUrlFmt?  Using CleanUrls almost *always* requires
explicitly setting $ScriptUrl, $PubDirUrl, and $UploadUrlFmt,
since PmWiki has no realiable way to determine what they should be.

    $UploadUrlFmt = 'http://www.theduncanfirm.com/uploads';
