Which are you looking for...
   1.  An edit template -- i.e., when a user edits a new
       page it comes up with some default text other than
       "Describe <<title>> here."  
   2.  A true HTML form with text fields and text areas,
       and when the user presses "submit" it creates a page
       from the data provided in the HTML form?

If #1, take a look at http://www.pmwiki.org/wiki/Cookbook/PageTemplate.
If #2, I don't think it exists yet but needs to be written (and could
probably be written without too much difficulty).

If something other than #1 or #2, I've totally misread what you're
hoping to achieve.  :-)
