It fails in the normal page display, while the edit preview is still ok.

Yes, it is. In fact I haven't tested without including preview.php.

I've stepped-back to version 0.5.20, and everything is ok now. If my
answers above are not enough to point you in the right direction I will
install 0.5.21 again.

Oh, before turning back to the old version I notice one thing: files in
wiki.d/ created/modified by 0.5.21 contained what it looked like an error
message: "No newline at end of file". Attached is one of those files,
namely PmWiki.SandBox. I hope this helps.
