[pmwiki-users] CommentBoxPlus: user defined forms

Does anybody have an example of how user defined forms work in CommentBoxPlus?

I've been trying to reformat the recipe, which uses fixed table, for
several hours now but some functionality always ends up broken. The
syntax of PHP with HTML tables and a form all in one, is really
difficult to deal with. So I'd rather use my own form in wiki page
syntax.

I guess I need to know which variables are required. I especially need
to add Captcha manually, so I can format it as required. I do *not*
want to enable it as a global requirement in config.php.
