Just to share a little fun with forms of my own:

It uses the goalmeter recipe and radio button voting.
The display of names and choices made could as well be hidden or not
produced.

The goalmeter could be changed into a vertical bar, and dimensions and
colours changed.

A short foxfilter script calculates the percentages, used as page
variables.
