It works for me, on the same Free.fr servers:

http://anamnese.online.fr/pmwiki-efo/index.php/Main/WikiSandbox

It's probably because of a missing or wrong .htaccess (see my previous
post about this).

The code is only:
(:html5video filename=test poster='' width=480 height=270:)

and test.ogv is in /pmwiki-efo/uploads

This cookbook should be modified: it tries to load the 3 supported
formats, which is not really efficient:

I'd prefer to explicitly write the extension myself because the file is
called test.ogv, not only test: (:html5video filename=test.ogv poster=''
width=480 height=270:)
