In looking at the ?action=diag output, it appears that you don't
have any passwords set:

    [DefaultPasswords] => Array
        (
            [admin] => 
            [read] => 
            [edit] => 
            [attr] => 
            [upload] => 
        )

What are you using to set the passwords?
