If no one has anything better, here's a hack I'd explore:

Create a local template ("titlespacedOnlyCatA") that contains a  
conditional that shows only pages assigned to CatA, then in your  
pagelist specify link=Category.CatB.

I don't know if that will work or work acceptably, but you asked for  
ideas...
