For commentboxplus you would need to define $CommentBoxFmt in
config.php. Copy the php code for $CommentBoxFmt from
commentboxplus.php, and modify it. Replace the EnableAccessCode part
with the captcha HTML code.

Sorry this is sketchy and needs to be filled out.

Another approach would be to use (:input:) markup to define a
commentbox, and include the captcha markup with it.
