[pmwiki-users] Implementing a survey

Hi all,

I need to publish a survey that's currently formatted as a word document.
My only constraints are : 
	1 - only registred users can take the survey ( that's an easy task ) 
	2- need to list all the survey that's been submitted ( just the users in a 
particualr group ) .
	3 - time : I need it the day before yesterday  :)

My ideas were  : 
	1 - extend the upload form so that the users could upload the survey compiled 
off-line :
		- the new form would name the file as <username>-<survey name>-<upload date> 
and place them in a dedicate directory
		- create a list of the uploaded files in that specific directory

	2 - create a form ( with all the survey question ) that would create a 
document ( the compiled survey ) 
		- the page would be named  as <username>-<survey name> in a dedicated group
		- the pagelist would simply be listing on the survey's group



What do you think would be the best way to achieve this ?
