Others have discussed it as well, but in all likelihood when
you updated to beta16 the new version of pmwiki.php wasn't
copied across.  

(Perhaps you had renamed pmwiki.php to index.php?  If so, see the 
first question in http://www.pmwiki.org/wiki/PmWiki/Troubleshooting .)
