In simple tables, one can generate rowspan by using empty cells ( |||| )

In advanced tables, colspan, rowspan, and any other table attribute
will work just fine.

We haven't been able to come up with agreement on the syntax for
header cells -- see http://www.pmwiki.org/wiki/PITS/00535 .
