There weren't any real steps to get from 0.6.x to 1.0.0 -- despite the
first number change, it was more to recognize that PmWiki had become
fairly stable than because there were any substantial changes.

So, to move from 0.6.x to 2.0 is basically the same instructions
as moving from 1.0.x to 2.0.
