As the instigator of some of the initial two column (implemented by 
tables) document index

the main rationale was to avoid (or at least reduce) scrolling.
I would posit that any page that doesn't (say) fit more or less on a 
reasonable sized screen (say 1024x768) should be a candidate for 
breaking up into smaller pages.

Given that the document index is an "Index" this doesn't seem 
necessarily the right thing to do,
but making better use of the space on the page (without making it too 
crowed) seemed the best thing to do.
