The markup in that example is possibly a little off.  It should be

    || border=1 width=100%
    || || || right column ||
    || || middle column ||||
    || left column ||||||
    || left column || middle column || right column ||

I suppose one could argue that in

    || border=1 width=100%
    ||||| right column ||
    |||| middle column ||||
    || left column ||||||
    || left column || middle column || right column ||

the leading empty cell for the first and second rows should be
assumed -- I don't have an issue with making this the case.
