Ok. It runs.

May be ther is an error in minimage.txt, where you write
  "include (cookbook/minimage.php);".
That's better with "include_once("cookbook/minimage.php");"

Another problem are the title, copyright and "special instructions". 
Where did they go ? I only have "Image collected on a PmWiki" and 
"Copyright:" under the image...
