I used Pm's example, it is adequate for now.

Regarding the other question, I found a way to move Preview before the edit
field in 
http://article.gmane.org/gmane.comp.web.wiki.pmwiki.user/3271/match=pageeditfmt

I also found $AuthorRequiredFmt and modified that.

This should help for now.
