I'm sure this is in the docs -- but, for the life, of me, I haven't 
been able to find it -- so I figured I'd as here:

I need to insert footnotes into a document -- you know, a 
superscripted number in square brackets[5] for which a reference is 
provided at the bottom of the page. What is the markup for doing this?

Apologies for asking something that I'm sure is patently obvious, but 
which I was unable to find in the docs.
