There's now a new version of HandyToC at
http://www.pmwiki.org/wiki/Cookbook/HandyTableOfContents

This enables the "don't include a H1 header if there's only one"
behaviour to be turned off.  It's still on by default, though.
I've also tidied up the code a bit.
