[pmwiki-users] UserAuth2 - No Login Form

Hi,

 

Last night I downloaded and configured UserAuth2 on my pc at home
without a hitch.  Now trying to do the same on my test server at work
but when I click Edit I'm not being prompted for my user/pass but
instead get something like below.  Can't seem to pinpoint my error, any
thoughts?  Thanks in advance.
