[pmwiki-users] unable to upload

I've been through the PmWiki documentation pages several times, checking
settings in config.php (see below), etc. and haven't been able to upload. My
site is running on Win2k3 Server and I've got the upload directory set as
writable.
Config.php currently reads:

$EnableUpload = 1;
$UploadDir = "uploads";
$UploadUrlFmt = "http://wiki.tomrue.net/uploads/";
$UploadPrefixFmt = '/$FullName';
$EnableDirectDownload = 0;
$EnableUploadOverwrite = 0;
$EnableUploadVersions = 1;
$UploadExts['jpg'] = 'image/x-jpg';

What am I missing? Any suggestions appreciated, with thanks.
