This should now be fixed. Get the latest version from
http://pmwiki.org/wiki/Cookbook/PageTableOfContents
It will also be in the next (2.0.0) release of PublishPDF.

New feature: show/hide toggle

The toc heading now includes a show/hide toggle. See an
example at
http://www.wikipublisher.org/wiki/index.php?n=Wikipublisher.FAQ
Click the "hide" link. Then click the 'show' link.
The recipe is written in a way that allows anyone to add
content toggling to a recipe or skin. Perhaps this should
be in the core -- eg I would love to hide the markup
quick reference when viewing a preview.
