Thanks to Patrick's help, I have repackaged quicktoc so that it is 
vastly easier to install. See 
http://www.pmwiki.org/wiki/Cookbook/QuickPageTableOfContents.

Patrick could you redeploy quicktoc for the cookbook page (and test area 
if you want) for me?
