How does PmWiki know what case to use in the page's name?  Yes, I know 
it's proper name gets stored in the page file itself, but where does that
proper name come from in order for it to be stored in the page file?
(This question is somewhat rhetorical, as I have my own answers to
this question but need to hear it from others in case I missed
something.)
