I'm confused; what do you mean by optional syntactic sugar? If "--" is
optional, how does PmWiki know where the link text ends and the title
begins? Eg:

[[Lorem ipsum dolor sit amet -> http://example.org]]

Or do you just mean that specifying a --title is optional?

Lastly, would it be possible to specify a title for other things, such as
images?
