Well my quick hack produces:

 December 11, 2004 at 10:06 AM by 192.168.0.1
 December 11, 2004 at 10:06 AM by Patrick Michaud (192.168.0.1)

which looks fine IMHO.

But this doesn't help if people want the IP address suppressed.
