Oh, sorry about that--I neglected a step.  You also have to be using
a pmwiki version 0.5.13 or later--there was a bug in 0.5.6 through 0.5.12
that didn't recognize Apache 2.0 filter as PATH_INFO.

You might also take a look at 
  http://www.pmichaud.com/wiki/PmWiki/ChangePmWikiURL
which describes a couple of methods for removing the pmwiki.php 
(or index.php) from the URLs, as is done on the pmichaud.com site.
